HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can access this information. Please review it carefully.

Our Commitment to Your Privacy

Interceptor Health is committed to protecting the privacy of your health information as required by the Health Insurance Portability and Accountability Act (HIPAA). This notice applies to health information generated through our screening program.

What Is Protected Health Information

Protected Health Information, or PHI, includes information about your past, present, or future physical or mental health condition, the screenings you receive, and related payment information that can identify you.

How We May Use and Disclose Your PHI

We use and disclose PHI for treatment, such as sharing your screening results with our clinical team and, where you have authorized it, your department's occupational health provider; for payment, to bill for services rendered; and for healthcare operations, such as quality review of our screening program.

We may also disclose PHI as required by law, for public health activities, or to avert a serious threat to health or safety, consistent with HIPAA's requirements.

Uses Requiring Your Written Authorization

Uses of your PHI not described in this notice, including sharing your results with parties outside your care team or department beyond what you have authorized, will only be made with your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

You have the right to request access to and a copy of your health records, request corrections, request a list of certain disclosures we have made, request restrictions on certain uses, and request confidential communications at an alternate address or phone number.

To exercise any of these rights, submit a written request to our Privacy Officer using the contact information below. We will respond within the timeframes required by law.

Our Responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
